Expressions of Hope Submission Form

In May 2015, the Conquer Cancer Foundation (CCF) launched The Campaign to Conquer Cancer, a $150 million fundraising effort to create a world free from the fear of cancer. The Campaign will increase CCF’s commitment to investing in research and science, patient education, and global oncology. The 2016 Expressions of Hope calendar will showcase The Campaign’s theme: Together, we’re taking down cancer. As you create your submission, consider the many ways one can conquer cancer and how you or someone you know and love is taking down this terrible disease.

Please complete this form and submit it with your artwork by July 31. If you are under the age of 18, a parent or legal representative must sign the form. Please also include contact information (mailing address, telephone number, email address) as we require it for our records. Your contact information will not be sold or shared with any outside parties. However, the rest of the information you provide below may be used and published by ASCO, Cancer.Net, and/or CCF in connection with your artwork. Paintings, drawings, mixed media pieces, and photos may be submitted. Please ensure that all submitted artwork is horizontally-oriented. If you are submitting a photo, please ensure that it is high-resolution (300 DPI). Fields marked with an asterisk (*) are required.
